Please bring your policy identification numbers if you want me to do so. Please note: You are responsible for the portion not covered by your plan. Do I need a doctor's referral to make an appointment?
While you do not require a doctor's referral to make an appointment, some extended health plans require one for reimbursement. These plans vary greatly in coverage and it is, therefore beneficial to contact your provider for details. A receipt will be issued to you for reimbursement.
